TWC All My Relations is committed to supporting our clients in achieving their independence after primary treatment completion. After care planning begins at approximately Day 45 when the client is asked to set post treatment goals. Program staff will assist the client with creating specific action steps on each goal that has been set. At the end of primary treatment, the client will have a comprehensive after care plan with clearly defined goals.
We believe that after care is a collaborative effort and we strive to include the client’s referral worker and other agencies the client is involved with in the development of an after care plan. After care plans usually include a combination of goals created by the clients and care plans developed with our partner organizations.
If a client normally resides in the Lower Mainland we will encourage them to reside in second stage after primary treatment to allow time for the client to adjust to independent living. If the client has a stable and supportive home environment, we will offer our support on an ongoing basis. Many clients will identify further counselling and cultural support in their after care plans and we will initiate those referrals in a timely manner.
Clients that are living outside the Lower Mainland and who desire to return to their home community will be supported in their transition. In these instances, program staff will work closely with the referring agency to ensure a smooth continuum of care. We arrange for follow up support with local organizations and program staff are available for individual support.
Alumni are welcome to attend program groups and cultural activities at TWC All My Relations. We also have a weekly support group for alumni both at TWC All My Relations and the main centre in Vancouver.
At graduation we make it clear to our new alumni that they will always have a home at TWC All My Relations.
